Required Documentation and Recommendation Forms

School Records

• Kindergarten Students: Not Applicable.
• 1st–11th Grade: Copies of current and previous school-year report cards and standardized test results, as well as any educational or diagnostic testing results.
• High School applicants must provide an official transcript displaying completed courses and credits.

Personal Records

• Copy of Birth Certificate.
• Student Photo (4×6 snapshots and phone photos are acceptable).
• Parent Questionnaire (completed online).
• Parent Testimony (completed online).
• Student Questionnaire (5th-11th Grade; completed online).
• If applicant requires classroom accommodations due to a diagnosed learning difference, please submit child’s current diagnostic testing, IEP, 504, or accommodation plan.
• If applicant’s parents are divorced, please submit the most recent copy of all court documents regarding custody and educational decisions that are notarized and signed by a judge.

Academic Recommendations (K-11; completed in Veracross)

• Kindergarten: early childhood teacher recommendation, school principal or preschool/daycare director recommendation
• 1st-6th Grade: teacher recommendation and principal recommendation.
• 7th-11th Grade: math teacher, English teacher, and principal recommendations

Character Recommendation (K–6; completed in Veracross)

This form must be completed by a non-family member such as a pastor, Sunday school teacher, special teacher (art, music, PE), childcare provider, coach, etc.

Pastoral Recommendation (7th-11th Grades; completed in Veracross)

If your child attends Middle School or High School ministries at Cornerstone Chapel, you can send your pastoral recommendation requests to the following individuals:

If your child does not attend Cornerstone Chapel, you may send your request to a pastor familiar with your child’s attendance at church and/or church activities.

Virtual Pre-Application Meeting (required for new families)

The purpose of our virtual pre-application meeting is to learn about your family, discuss our admissions process, and determine if CCA is the right fit for your child’s educational needs. Enrollment eligibility and grade level openings will determine the number of applications processed after this step.

Application Fees and Admissions Testing

Application Fees
A $100.00, non-refundable application fee is due at the time of the application submission and entrance testing for grades K–11.

Admissions Testing (By Invitation)

• K–2 applicants will complete an admissions assessment administered by our team of professional educators. Testing is $150, paid at the time of scheduling.
• Students entering grades 3–11 will take the iReady admissions assessment administered by our team of professional educators. Testing is $150, paid at the time of scheduling.

Family Interview (By Invitation)

• Once the application file is complete, we will contact parents to schedule a family interview.
• This interview is an opportunity for philosophical compatibility discussions and a question-and-answer session with members of our school leadership team.
• All applicants must participate in a family interview, regardless of grade level.
