Required Documentation and Recommendation Forms
School Records
- Kindergarten Students: Not Applicable.
- 1st–10th Grade: Copies of current and previous school-year report cards. Standardized test results as well as any educational or diagnostic testing results.
Personal Records
- Copy of Birth Certificate.
- Student Photo (a 4×6 snapshot or a picture from your phone is acceptable).
- Family Photo (a 4×6 snapshot or a picture from your phone is acceptable).
- Parent Questionnaire (completed online).
- Student Questionnaire (5th-10th Grade; completed online).
- If applicant requires classroom accommodations due to a diagnosed learning difference, please submit child’s current diagnostic testing, IEP, 504, or accommodation plan.
- If applicant’s parents are divorced, please submit the most recent copy of all court documents regarding custody and educational decisions that are notarized and signed by a judge.
Teacher Recommendations (Completed Online)
- Kindergarten: early childhood teacher recommendation, principal recommendation ( pre-school/daycare director)
- 1st Grade: early childhood teacher recommendation, principal recommendation.
- 2nd-6th Grade: teacher recommendation and principal recommendation (of current or previous year).
- 7th-10th Grade: principal, math, and english teacher recommendations.
Pastoral Reference and Character Recommendations (Completed Online)
Every applicant is required to submit a character reference recommendation and a separate pastoral reference attesting to the applicant’s walk with the Lord.
If your child attends Middle School or Children’s Ministries at Cornerstone, you can send your recommendation requests to Pastor Ken or Pastor Barrett. If not, you can send your request to a pastor familiar with your child’s attendance at church and/or church activities.
In addition, each applicant is required to provide a completed character recommendation. This form must be completed by a non-family member such as a coach, Sunday school teacher, tutor, specials teacher (art, music, PE), childcare provider, etc.
Pre-Application Virtual Interview (By Invitation)
The purpose of our virtual pre-application meeting is to learn about your family, discuss our admissions process, and determine if CCA is the right fit for your child’s educational needs. Eligibility and space will determine the number of applications released.
Fees & Testing (By Invitation)
Application Fees
A $100.00, non-refundable application fee is due at the time of the application submission and entrance testing for grades K–10.
Admissions Testing
- Kindergarten and 1st Grade: Applicants will take a placement test given by our admissions office. The cost for testing is $150.00 paid at the time of scheduling.
- Students entering grades 2–10 will take the Independent School Entrance Exam (ISEE) online. The exam cost is determined by ISEE and varies depending on testing location and lead time.
Financial Aid (Optional)
CCA uses FACTS Financial Aid to assist with financial aid assessments. FACTS allows for a consistent approach in determining tuition needs. Families should provide financial information and answer all questions to give the financial aid committee a comprehensive view of the family’s overall need.
We understand the challenges associated with making this level of financial investment in your child’s education. We encourage you to prayerfully consider your circumstances and to determine the best course of action for your family. CCA is “admissions blind” and makes enrollment decisions independent of financial aid requests. Following your financial aid decision, you will have five days to finalize your enrollment decision.

Admissions Decision & Enrollment
Admissions decisions will be based upon:
- Partnership and alignment with CCA’s Mission, Values and Beliefs
- Strong recommendations
- Acceptable entrance testing scores
- Grade-level availability
Enrollment:
- Sign enrollment contract
- Select FACTS tuition payment plan
- Pay $250 enrollment fee (per child)
- Pay $250 enrollment deposit (per child)
